Health Self-Screening Questionnaire

Please answer the following questions before coming to the arena. You will be asked these questions again when you arrive for your session.

The answer to ALL questions must be NO in order to participate in each on-ice activity.

Do you have any of the following symptoms?

  1. Fever? (Feeling hot to the touch, a temperature of 37.8C or higher)
  2. Cough (that’s new or worsening)
  3. Shortness of breath
  4. Runny, stuffy or congested nose (not related to other known causes such as seasonal allergies etc.)
  5. Sore throat
  6. Difficulty swallowing
  7. Lost sense of taste or smell

Travel or Close Contact?

  1. Have you travelled outside of Canada in the past 14 days without a Government of Canada Travel Exemption*?
  2. Have you had close contact in the past 14 days with anyone with a confirmed case of COVID-19, without the consistent and appropriate use of personal protective equipment?

Please note: This Health Screening questionnaire has been developed based on the current Ontario Ministry of Health Self Assessment Tool.

CanSkate B COVID-19 Safety Protocols

Before Coming to the Arena
  • All skaters must pass the Health Self-Screening before coming to the arena.
  • If you or your child is not feeling well YOU MUST STAY AT HOME.
Arena Entry – FRONT Doors or WEST Doors
  • Skaters should arrive at the designated entry doors 15 minutes prior to their session start.
  • TUESDAY, THURSDAY and FRIDAY 5:15 will use the FRONT DOOR for drop off and pick up.
  • FRIDAY 6:15 will use the WEST DOOR around the side of the building for drop off and pick up.
  • Entry doors are alternated with other sessions to allow for sanitizing of all surfaces.
  • Skaters MUST wear a mask when in the arena. The mask can ONLY be removed when on the ice.
  • Individual health screening and hand sanitation will be done at the door.
  • Entry doors will be locked when the session starts. Please make sure you arrive on time!
  • NO spectators are allowed in the arena. If there are any issues that need your attention we will call you.
  • Follow the social distancing markers on the ground outside the arena door when waiting to enter.
  • Skaters should arrived dressed with skates and helmet ON. Skates should have guards on to protect the blades.
Personal Belongings
  • Do NOT send skaters in with any personal belongings.
  • Skaters should be wearing their mitts, snow pants, jacket, helmet and skates when they arrive.
  • Parents/guardians will not be allowed in the arena to tie skates.
Pre-Ice Entry
  • Once in the arena, skaters will be escorted to a social distancing chair provided for them in the hallway.
  • Volunteers will be on hand to help skaters maintain social distancing.
Ice Entry and Storage of Personal Belongings
  • Skaters will be escorted to the ice one group at a time.
  • Skaters will use the same doors for ice entry and exit to avoid interacting with the next group of skaters.
  • Skate guards must be placed in the individually designated spaces marked on the rink boards.
On Ice
  • Skaters can wear a mask while skating but it is not required.
  • Coaches are required to wear a mask while coaching.
  • Skaters and coaches must social distance on the ice.
  • Coaches will not touch the skaters except in the case of emergency.
  • Skaters must stay on the ice and not use the benches in the penalty boxes.
Ice Exit
  • At the end of the session, skaters will gather all their belongings and be escorted off the ice with social distancing.
  • All skaters must sanitize their hands before exiting the arena.
  • Skaters will be escorted to the door for pickup by their parent/guardian. This will be the same door where the skater entered for their session.

Parent & Tot COVID-19 Safety Protocols

Before Coming to the Arena

  • Skaters and parents/guardians MUST wear a mask when in the arena. The mask can ONLY be removed when on the ice.
  • Individual health screening and hand sanitation will be done at the door.
  • Entry doors will be locked when the session starts. Please make sure you arrive on time!
  • NO spectators are allowed in the arena.
  • Follow the social distancing markers on the ground outside the arena door when waiting to enter.
  • Parents/guardians may put their skates on in the arena.
  • Skaters should arrived dressed with skates and helmet ON if possible. Skates should have guards on to protect the blades.
Personal Belongings

Please bring as little as possible into the arena. Suggested items:

  • Skates with guards.
  • Water bottle labeled with skater’s name.
  • Tissue box and your own zip lock bag for used tissues.
  • If you need to bring a small bag, you must keep all of your belongings in the bag and leave it beside your designated chair.
Pre-Ice Entry
  • Once in the arena, you will be directed to a social distancing chair in the hallway.
  • If you enter through the west door, chairs are located in the area around the snack bar.
  • If you enter through the front door, chairs and benches are located in the hallway outside the coaches room.
Ice Entry and Storage of Personal Belongings
  • The parent/guardian will escort their skater to the ice through the marked ice entry door following social distancing markers on the floor.
  • West door entry skaters will use the south west corner ice entry door.
  • Front door entry skaters will use the north ice entry door.
  • The same ice doors are used to exit the ice to avoid interacting with the next group of skaters.
  • Water bottle, guards and tissue box must be placed in the individually designated spaces marked on the rink boards.
On Ice
  • Skaters and parents/guardians can wear a mask while skating but it is not required.
  • Coaches are required to wear a mask while coaching.
  • Coaches must social distance on the ice from all participants.
  • Coaches will not touch the participants except in the case of emergency.
  • Skaters must stay on the ice and not use the benches in the penalty boxes.
Ice Exit
  • At the end of the session, parents/guardians will gather all their belongings and will exit the ice with social distancing.
  • The parent/guardian will escort their skater back to their designated chair / bench and get ready to leave the arena as quickly as possible.
  • All skaters and parents/guardians must sanitize their hands before exiting the arena.

Before Coming to the Arena

  • Entry doors are alternated with other sessions to allow for sanitizing of all surfaces.
  • Skaters MUST wear a mask when in the arena. The mask can ONLY be removed when on the ice.
  • Individual health screening and hand sanitation will be done at the door.
  • Entry doors will be locked when the session starts. Please make sure you arrive on time!
  • NO spectators are allowed in the arena.
  • Skaters should arrived dressed with skates on if possible.
  • Follow the social distancing markers on the ground outside the arena door when waiting to enter.

  • Once in the arena, skaters will be directed to a social distancing chair provided for them in the hallway.
  • If you enter through the west door, chairs are located in the area around the snack bar.
  • If you enter through the front door, chairs and benches are located in the hallway outside the coaches room.
  • Locker room entry is restricted to one skater at a time. Please knock and ask if anyone is in the room before entering.

  • West door entry skaters will use the south west corner ice entry door.
  • Front door entry skaters will use the north ice entry door.

  • Water bottle, guards and tissue box must be placed in the individually designated spaces marked on the rink boards.
On Ice
  • Skaters do not have to wear a mask while skating.
  • Skaters and coaches must social distance on the ice.
  • Coaches are required to wear a mask while coaching.
  • Skaters must stay on the ice and not use the benches in the penalty boxes.
  • Skaters can not leave the ice without telling their coach.
  • Music will be played from the music room most of the time.
  • Skaters must sanitize their hands before using the music request intercom box.
Ice Exit
  • At the end of the session, skaters will gather all their belongings and leave the ice with social distancing.
  • Skaters must go back to their designated chair / bench and get ready to leave the arena as quickly as possible.
  • Skaters have 10 minutes after their session is finished to leave the arena before the door is locked.
  • All skaters must sanitize their hands before exiting the arena.
